All cancers need to be staged and graded when you first get the diagnosis. This is done generically by biopsy, bloodwork, imaging. Many cancers are staged with a "TNM" classification and range from 1-4. As explained by some of the other posters, when the cancer begins to invade and penetrate past the initial primary organ into the bloodstream it essentially 'seeds' somewhere else, or what is called metastasizes (the M in TNM). Once we have metastasis, you are largely classified as stage 4. The reason why transplant is not an option is because that is a high risk procedure for a high demand organ for a patient who is, to not mince words, terminal. Surgical resections of the tumor can be sometimes done but on the liver it is also difficult so chemotherapy can help to reduce the cancer burden of the body and hopefully slow down progression. The issue is that all patients who have metastasized will continue to do so, whether the liver has more 'spots' or they begin to occur in the bones, brain or lung. From my understanding, if the cancer spread it could have spread elsewhere too while being undetectable. So a cancer patient wouldn't be anywhere near the top on the transplant waiting list because the odds are too high that the cancer is just elsewhere and would spread right back the the liver, thus "ruining" the organ. That all could be wrong. I ain't no doctor. But I think it's really because they want to give the organ to someone who's otherwise healthy. Plus there's probably a thing about how it's a major operation and those people want to be on chemo. Chemo + needing antibodies to combat infection is dangerous. Source: House | ||
