It was close, just a lot of botched decisions...

Gogoing tryinig to run away in 2 teamfights and not having zhonyas first cost them their early lead. He even flashed into his own adc with fizz ult on him.

I think they tossed away the chance for an even match at the aforementioned dive. It was a mistake, but it isn't on the level of what "throw" usually means on here. They took a big gamble on trying to win a dive before Trist got too big and it didn't pay off. It was a terrible idea in hindsight, but I can see what LD was going for there and conceptually trying to blow up Trist with Fizz and having the team jump in wasn't the worst of ideas. The execution just wasn't there.

But yeah, erm... bad Zorozero! (I mean, he did screw up, but the avalanche was not started by him)

Bigpomelo with the clutch catches into Zyra+trist combo. Feel like mithy didn't give Zyra+Trist lane the respect it deserved and played too far forward. they should win, just the catches really screwed over the lead that level 1 gave Corki.
