Nobody's completely sure, but it seems to spawn things that match the area. Personally, I get the dratini line pretty much anywhere there's a body of water, but only very occasionally. I'd say it's likely they're in your area even if you've never seen one.
I think your friend just got absurdly lucky. Personally, I've never had high value pokemon spawn from incense/lures at all, it's usually just pidgeys/zubats/other trash.
Caught a random meowth walking along the road today though that was weird on the way to work haha. Fun game
