Let's replace some words to make it easier:
The fifteenth ill sheik's ninth sheep is sick.
The first sixth is the number of the sheik, the second one is the number of the sheep, like Iron Maiden's album "Seventh Son of a Seventh Son." However that album was by a good margin their worst, so that may not be a good analogy.

yeah basically it means there's at least six sheiks that are sick. and the sixth one has at least six sheep, the sixth of which is sick
