Just a terrible accident. I think it's silly to blame anyone for the placement of the vent or anything like that. It's pretty clear that you're not supposed to climb on top of vents, and I'm sure even the people who climbed on it knew that before they did. Overzealous fans doing something risky in an attempt to enjoy the performance better who made a tragic mistake :/
