No way he forgot, I mean forgetting in one panel can happen if your editor is having a bad day or whatever, but Shunsui was drawn without the eyepatch in every panel he was in (~5), plus the cover art had him WITH an eyepatch. So yeah that seems incredibly unlikely.
While I think the Aizen theory is very...unlikely, I do think there's some reason for losing the eyepatch...and Shunsui is specifically stated to dislike tea (as someone mentioned he always drinks sake and eats steamed buns)...and tea was Aizen's thing...so yeah idk. I'm withholding judgement lol.

Nanao would pick up on something like that... it's way too obvious to do it in front of her... it's more likely shunsui making a deal with aizen. I think that is more likely as Shunsui has always seemed like a character that questions norms and traditions and uses any means necessary to win. It would be very like him to make a deal with aizen in exchange for freedom in the same way he OK'ed Zaraki's training.
