Some hints:
- you get the HD stream on the site where you bought the pass (esl or twitch) - and can watch it only there
- the hd stream is the same one... the esl site uses the twitch hd stream
- hd should be 1060+, on esl it only shows 720+ but there is no difference to 1060+ on twitch

Last time there were 2 or 3 hours delay because of a ddos attack i think.

Go to the HD twitch.tv page and select the 720p+ resolution it will ask you to login (& ofc buy) a pass
