This was in the 1980's iirc (might have been 1990's) and it was a joint venture with the Soviet Union/Russia to send a probe to Mars. Americans read the Soviet measurements as inches when they were in fact centimeters (or miles/kilometers, same principle), and ended up launching the probe too hard and it just exploded on Mars. GG.
