A lot of times by pikachu, the first time I think is when he first gets pikachu, and then some times in the woods etc.
Haven't seen a lot of them since I was generally too old to watch them when they aired so I could be wrong.

To elaborate, you will notice that pokemon red was made before the first episode, they simply added a new spin to it with pikachu following him which is where pokemon yellow came in. After that it all is just randomness.
I read the whole thing and admittedly it does make sense but I still have no doubt that they simply have over-analyzed a show based on a video-game.

Then Misty got pissed off at Ash for destroying her bike, hence why she followed him in the first place.
