I suggest TES IV: Oblivion or TES: III Morrowind but it's a bit older. It's bias cause I played Oblivion first. But c'mon TES are amazing.
Morrowind is great.
Oblivion kept all of the flaws that Morrowind had, removed much of its greatness, added some stupid shit like leveled gear and random bandits with daedric equipment and then had ~three voice actors voice every npc in the game.

Another recommendation for Baldur's Gate. It was difficult for me, but I was 10 years old so I guess most games were difficult.
But if you were really hardcore you'd play roguelike games such as Angband.

Actually Deus Ex on realistic mode is still quite easy. For example, upgrade sniper rifle and just kill everything before they even notice you are there. You need to play Deus Ex with handicaps like crow bars only or no upgrades.

Not really a full fledged RPG, but it has some RPG elements in there. Just make sure to crank the damage sliders to "Normal" which is the hardest.

My personal favorite is ADOM, and there are many more others, even something like good old Doom rougelike for example (it's awesome).

I never thought RPGs were supposed to be hard, after all, they want people to experience everything. Most of my experiences come from consoles, PC's rpgs appear to be more difficult, like Baldur's Gate, Morrowind, Ultima series, etc.
I heard the Might & Magic series can be quite difficult if you can stay away from the exploits.
A game you haven't played but that's quite easy is Star Wars: Knights of the Old Republic. Just AMAZING.
And, finally, in the turn-based strategy games (you played X-Com, the best there is) you can try Jagged Alliance and Mission Force: Cyberstorm (a personal favorite of mine, buy mechs, customize them, clone a pilot and drug him! Ah, the joys of gaming before the political correctness squad ever existed) . I also heard nice things about Master of Magic.

I do this all the time, Phantasy Star IV without using inns, Skies of Arcadia without buying equipment, etc. If you look at Realms Beyond Diablo that are tons of these "challenges", some silly and some are really interesting, like the Amazon and Gothic for Diablo 1. Final Fantasy Tactics fit in this too.

Vagrant Story or Final Fantasy Tactics? Both by the same director from Square (the one that directed FFXII). Tactics is difficult because of the enemy leveling and that it takes more than powering to win, and Vagrant Story has a pretty complex character system going on.
I seriously need to be playing Baulder's Gate though.

Also I recommend Transcendence, it's not really a fantasy rpg but more in the style of Escape Velocity Nova (which is also good if you haven't tried it). you're in a spaceship and you kill other ships for loot and visit space stations instead of towns. same rpg formula but with a spaceship instead of a character.
